Screening
Screening is a requirement for all. It is so I know who I am meeting and feel safe in doing so. Rest assured, your personal information is for my eyes only.
Complete one of the options on the following form.
➀ References
If you have met with other providers in the past, simply state their names, website or advertisement links, and contact information.
➁ Identity Verification
If you choose this option, you’ll need to share something with me that verifies your face + full name. Most common options: LinkedIn profiles, a bio or profile on your company’s website, passports, driver’s licenses, or other types of ID. Send photos of ID or passports to misscarmenpaige@gmail.com
➂ Other
I will occasionally accept other forms of screening such as P411 information. Fill out the form with screening information you feel comfortable providing and I will let you know if it is sufficient or not.
